Delegating tasks or outsourcing for specific areas does NOT mean you also transfer the ownership of the outcome.

Sure, you delegate responsibility to someone and you BELIEVE they will do a job well because you checked their skills and agreed on terms and conditions.

BUT nothing is 100% sure ever. People are NOT machines and do not operate by algorithms and even when there are clear procedures, circumstances are NEVER completely the same.

Therefore, when mistakes happen, it’s never one person’s fault and blaming someone doesn’t do much. But this doesn’t mean you shouldn’t investigate and have LESSONS LEARNT and your systems improved through new PROTOCOLS.

Omission is always a result of many participants in a process failing to deliver fully. Someone was just the last one in delivering so they take the blame when in fact EVERYONE involved in a process failed in noticing, acting upon, double checking or communicating something.

This means that you as a main stakeholder need to take OWNERSHIP of such a situation. You should not hide from it, blame something or someone, ignore it or have someone fired.

NONE of the things above will solve your problem. It will keep happening.

Don’t point your finger at anything or anyone. Just observe, own it, take notes and fix it so it doesn’t happen next time. Because if you don’t, it will.

Don’t distance yourself from a problem. OWN it. Just like you do with a positive outcome.
